FY 2025 Mini-Grant Application
Applications including all supporting documentation are due July 31, 2025 at 4pm CST.
This mini-grant is available to individuals or organizations who can clearly demonstrate a need for support and propose services that align with Exhibit E: Opioid Remediation Uses. These services must be targeted toward residents of Morton and/or Mercer Counties.
The period of performance for this mini-grant is September 1, 2025 through May 31, 2026. Activities must occur within this time frame.

Application Information
Applicants may represent one of the following: community coalition or community-based organization; school or school district; behavioral health provider; substance use recovery, treatment or service provider; harm reduction provider; primary medical care; pharmacy; emergency or urgent care; law enforcement/corrections; first responders; residential and/or transitional living housing; and military or veteran services.
Projects will be selected based on the number of applications received, funding availability, and alignment with program priorities. Funding may not supplant any other local, state, or federal funds.
Applicants can submit proposals for one or all of these initiatives. Funding per organization will be capped at $5,000.

Funding will be awarded based on the completion and submission of grant deliverables. Recipients must submit documentation demonstrating that all proposed activities and objectives have been completed as outlined in their application. Grant payments will be issued as a one-time disbursement at the end of the grant period and will reflect either the full or a prorated amount of the approved budget, depending on the extent to which deliverables are fulfilled.
